Manor and The Finley Hospital for the purpose of designating Sunnycrest Manor as an alternate care site for patients of Finley Hospital in the event of an evacuation emergency at Finley Hospital.
Motion by Manternach, seconded by Smith, carried unanimously to approve and authorize the Chair to sign Resolution 09-181.
BE IT RESOLVED that the Dubuque County Board of Supervisors approves and authorizes the Chairperson to sign the Vocational Services Agreement between Sunnycrest Manor and Area Residential Care, Inc. for ICF/MR vocational services and related rates for the period from July 1, 2009 through June 30, 2010.
Motion by Smith, seconded by Manternach, carried unanimously to approve and authorize the Chair to sign Resolution 09-182, subject to review of the rates and a report back to the Board by CPC Jody Jansen.
WHEREAS, Dubuque County has been presented with an Environmental Review Certification for the Disaster Recovery Housing Fund Program, 08-DRH-004 and the Jumpstart Small Business Disaster Recovery Financial Assistance Program, 09-JMPST-11.
THEREFORE, the Dubuque County Board of Supervisors hereby approves and authorizes the Chairperson to sign the Environmental Review Certification for the Disaster Recovery Housing Fund Program, 08-DRH-004 and the Jumpstart Small Business Disaster Recovery Financial Assistance Program, 09-JMPST-11.

WHEREAS, Dubuque County has been presented with a Qwest Centrex Prime Rate Stability Service Agreement and Qwest Total Advantage Monthly Assessment Agreement for Dubuque County telephone services for a twelve month period.
